- Biosketches (curricula vitae, resume) for each member of the committee
- Cover letter for documents indicating the information is being submitted as the IBC’s annual report

To comply with the National Institute of Health (NIH) Guidelines for Research Involving Recombinant DNA Molecules (NIH Guidelines)
