What Should be Reported?

Pledged revenues, number of bed/units in total and number occupied, schedule of current rent changes in project/pledged facilities per student, itemized costs or comments (if required), Debt service coverage, status of applicable fund accounts, enrollment for each term for past school year, enrollment for past three years as reported to HEW/OE, bank names, addresses and phone numbers, other facility loans

When Should it be Reported?

Annually, after audit is complete. Usually started in November
