  • An updated committee roster, indicating contact information and the role of each committee member (e.g., community member, chair person, etc.)
  • Biosketches (curricula vitae, resume) for each member of the committee
  • Cover letter for documents indicating the information is being submitted as the IBC’s annual report

Annually

electronically via the Institutional Biosafety Committee Registration Management System

Why Should it be Reported?

To comply with the National Institute of Health (NIH) Guidelines for Research Involving Recombinant DNA Molecules (NIH Guidelines)

If Notification is Required, How is it Reported?

Office of Biotechnology Activities must also be notified if any changes in committee membership are made when they occur
